'Why' is so hard question.
The reason why I do with family is that my birthday is not only my anniversary. That also could be special day for my parents because my birthday means the day when they got their son.
I loved my birthdays during childhood until probably becoming a teenager, when exstential worries appeared (like having 'the person' to celebrate my birthday with). I used to celebrate birthdays with my friends and parents/brother/grandmother. Now its only parents/brother/grandmother. Sometimes also with friends, but most usually I spend the actual birthday alone, celebrate it with my friends a few weeks before, and celebrate it with my family a week after.

Usually I celebrate with my relatives and friends but it depends on the situation - sometimes I am abroad at the time of my birthday, other times a lot of my family members or friends are unable to make it to my birthday. I don't make a big deal out of it, as I usually never plan and specifically invite people - I just prepare a cake and some snacks in case people come and just go with the flow. I do like to be with people close to me on my birthdays because it makes the day feel special - otherwise it just feels like a regular day.

Usually 'celebration' is just a casual dinner with family, sharing the cake and wine. Also, I like to treat my best friend dinner or a night out as a thank you because she really prepares me gifts. This year I celebrated my birthday in Korean way with seaweed soup which was awesome. I do not think birthday is a big thing in general, that is why all my celebrations are only between me and very close people to me, more of small usual gathering.
